Care Management
When doing care management, we assess a client’s current situation, establish a care plan, implement that plan, and then provide ongoing monitoring, guidance, advocacy, and resolutions. We pride ourselves in offering knowledgeable and experienced support for any circumstance that should arise. We provide on-call support 24 hours a day, 7 days a week, to respond to any crisis. The goal of the team is to prevent those situations by providing dependable care management on a consistent, regular basis. We stabilize any immediate issues with our clients, improve their quality of life, and provide on-going peace of mind and solutions for their families. We are very flexible with our level of involvement. As needs arise, we will step into action more, but can also back off while things are stable. We are also often utilized for consulting and coaching families through the process.
Some of our clients do not have family, their family may live out of town, or may be busy juggling careers and children. Whatever the reason, we are available to be the professional family member you can lean on in your time of need. Essentially a privately hired social worker, Aging Life Care Managers function as an advocate, a problem solver, a resource, a confidant, and an emotional support for families. Our work is highly varied because our clients’ needs are varied, but could include:
- Coordinating other service providers
- Weekly or monthly visits to the home or facility
- Screening and training caregivers
- Medication management in the home
- Patient advocacy at physician visits and hospitalizations
- Communicating updates to family and professionals
- Facility placement and move management
- Dementia care support
- Crisis Intervention
Whether it is short term or long term, we provide you with answers, manage your challenges, and to step into action when you need professional support. All of our care managers are members of the Aging Life Care Association and are held to their Standards of Practice and Code of Ethics. Our goals are to improve quality of care and quality of life and provide peace of mind.
